Dr.
Woolley’s Retirement
As of June 30, 2007 Dr. Janice Woolley is no longer
with Puget Sound Behavioral Medicine. Health considerations
required that she retire from the practice of medicine. She was
diagnosed with multiple myeloma, a bone marrow malignancy. The
necessary chemotherapy and effects on her stamina were not
consistent with continuing an active medical practice.
How to arrange for your ongoing care:

How to request transfer of your medical records:
If you will be continuing your care with someone other than Dr.
Mandelkorn you will need to request your medical records to be
transferred to the new provider. Please go to
http://www.psbmed.com/forms.php4
and
print a copy of the medical request form. (item 6.) After
completing this with the full name and address of your new
provider and your will need to sign it and either fax it to
Puget Sound Behavioral Medicine at 206-275-0702 or mail it to
Puget Sound Behavioral Medicine, 2554 76th Avenue SE,
Mercer Island, WA 98040. Parents or legal guardians should sign
the form for children under the age of 14. Teenagers 14 years of
age and older need to sign the form themselves.
